What service do we provide?

RTH provides Case Management services for all programs. Case Management services include the following: Initial client interview/intake, short and long term goal setting

  • Weekly home visits and monthly apartment inspections
  • Regular group sessions, parenting groups, referral & advocacy with other agencies as needed
  • Financial planning and budgeting

Who provides the service?

The Case Managers of Range Transitional Housing, Inc. provide the Case Management service.

Where is the service delivered?

The services are provided on site, in the client’s home, in community rooms, or located at other facilities as needed.  Case Managers provide transportation as needed to other provider locations.

When is the service delivered?

Service is provided when needed but usually occurs from the hours of 8:00 A.M. through 7:00 P.M. Monday through Friday.  Case Managers begin working with the families when they are in the shelter or when a referral is received at the RTH office.  The Case Managers and participants schedule in-home visits in advance usually once a week.

How is the service delivered?

RTH staff tries to adapt to the style of learning that best fits the clients’ needs.  Services may be adapted to client’s specific needs, i.e. Braille, large print, handicapped rental units, etc.  During initial contacts, participants may need rides to connect with appropriate resources such as employment offices, social services, and the area food shelves.
